Wellness Assistance Program (WAP) 

Through our WAP, during COVID-19 Pandemic T.M.M.C. has & is still providing P.P.E. and other safety items to individuals in our community! Please see our Love In Action(1) tab.

Wellness Assistance Program (WAP) is a nonprofit patient assistance program established in 2020 by Team Miracle Man Charity. The chief aim of the program is to improve health outcomes by increasing access to critical healthcare for low-income, underinsured, and uninsured Georgians who cannot afford care.  

​

Services include a coordination of advocacy and referrals for medical treatment and pharmaceuticals.  We offer patient advocacy and negotiation to help settle issues associated with access to care and debt related to illness. Through early intervention resources, we are able to help clients with conditions that could very easily become critical, leading to emergent care visits at local hospitals due to lack of early diagnosis and treatment.  For individuals with life-threatening and chronic illnesses, we offer comprehensive and holistic support as well as referral services to assist them with spiritual, emotional, and educational support.

Service Descriptions

 

Advocacy

 

Advocacy involves increasing our client’s access to healthcare treatment resources and support and increasing community awareness of low-income, underinsured and uninsured population needs.  It includes advocating for client healthcare screening and evaluation, ensuring service fees are equitable and affordable, and offering debt resolution negotiations where appropriate. Proper educational resources offer opportunities to demystify myths and promote an accurate picture of healthcare access issues, risk factors, prevention methods, and solutions that reduce stigmas and open the doors to saving lives.  Through advocacy, we engage in beneficial conversations with clients and providers which encourages a pollination of ideas and strategies.  Advocacy also motivates everyone to do their part to ensure that we keep the low-income, underinsured and uninsured populations healthy – regardless of their ability to pay for care. 

 

Case Management

 

Case management is a central service that includes assessment of clients needs, development of individualized service plans, assistance in accessing publicly funded programs, health and wellness planning, information and referrals.

 

Case management also includes following up with clients post-services to ensure that their needs were addressed after services have ended.
 

Coordination of Resources and Referrals

 

Coordination of resources and referrals are offered to support our clients with access to critical healthcare and community-based services.  We use our client needs-assessment to identify services needed to match clients to medical service providers and trauma-informed community providers.  Community-based programs serve as a cost-effective way to close the gap in access for trauma-informed care.  To that end, clients receive appropriate resource information (based on their level of need) to local providers for legal, housing, financial, food, clothing, childcare and other social services.  Local providers help foster a sense of connection and belonging, as they provide critically needed services. 

 

Provider Services

 

The WAP program works to provide information about healthcare providers throughout Georgia to assist in securing charity-care services for people who otherwise would not have access to the treatment needed for their level of care.  Clients receive information for their condition and overall health, and linkages for continued care.

When the primary reason for each patient’s charity-care visit has been resolved, additional providers may aid them with self-care management strategies, such as education for their specific condition, and next steps in maintaining health and wellness.  Although this phase of care is designed to transition services from urgent care to local routine care, this transition is important to the long-term health of our clients.  After-all, the goal of health care providers, such as hospitals and doctors, is not just to make our clients well when they are sick, it’s to keep them from getting sick initially.

 

Trauma Informed Services

 

In addition to health emergencies and treatments, our clients often have other life issues brought on by poverty, joblessness, homelessness, drug abuse and more. We use a compassionate approach to understand our patients’ needs and then connect them with resources in the community to meet their needs. 

 

Community Health Fairs (COVID-19)

 

Because safety is vitally important to us, during the coronavirus, we (will not) host any event that requires close contact, for the safety of all; however, as a part of our advocacy and outreach work, we will engage in community health fairs so that we have an opportunity to take care of those who are underserved, underinsured or uninsured.  During health fairs we will provide health and nutrition education and we will partner with health care providers to deliver an interactive experience for attendees that include basic preventive medicine and medical screening.

 

Financial Assistance

 

Subject to funding, we provide financial assistance to help clients with:

  • Prescription copays

  • Health insurance premiums, deductibles and coinsurance

  • Pediatric treatment costs

  • Medical related travel costs

Durable Medical Equipment (DME), and Medical Supplies:

• Canes

• Crutches

• Walkers

• Rollators

• Bedside Commodes

• Tub Transfer Benches

• Shower Chairs

• Manual Wheelchairs

• Transport Chairs

• Adult Incontinence Items (adult diapers, bed pads)

• Face Masks

• Gloves

• Ointments  

• Bandages

 

Rendering of Services

 

  1. Referrals of medical, dental, and mental health treatment providers. 

  2. Referrals of trauma informed services by community-based providers.

  3. Patient advocacy, case management, provider referrals and follow-up, nutritional wellness and healthy living education information is provided directly by the WAP program.

 

Enrollment

 

All prospective WAP clients must complete an application for services via mail or phone. There is a determination of program eligibility and assessment of needs completed by our team.  Once the eligibility process is complete, approved applicants enter case-management where they receive a comprehensive needs assessment before being referred.  If an individual is not eligible for the services of our WAP we consult with them explaining the reason for denial; we then offer information of other community-based providers that may be able to assist them. 

 

Location and Hours

 

The WAP program is headquartered in Alpharetta, Georgia.  Program referrals, advocacy and educational services which are delivered by the WAP team may be rendered virtually using video conferencing, email, phone, text and our website.

 

Our business office hours are Monday – Friday from 9:00 am to  5:00 pm. We offer after-hour assistance for urgent care situations.  Virtual information is provided 24/7 via our website.
